| a. Increase enrollment in Special Care Physician Case Management Program.
b. Target enrollments for clients that would be most affected by the limited mental health benefit levels offered under the traditional Medicaid Managed Care Plan.
c. Incorporate findings and recommendations from Special Care Physician Case Management Program into other Managed Care Initiatives.

a. Identify and target additional high-risk/high Medicaid utilization drug and alcohol clients, particularly those receiving Family Assistance or Medicaid only.
b. Refine service model which discourages repeated inpatient admissions for detoxification or rehabilitation services and emphasizes ongoing outpatient follow-up care.
c. As part of the project, provide a case management approach to ensure outpatient follow-up and the coordination of medical care and special care.
d. Develop and implement new outpatient service alternatives in the areas of detoxification and rehabilitation.
e. Expand managed care strategies to high users of other behavioral health services.

a. Refine, enhance and expand single point of entry and care coordination strategies in the delivery of mental hygiene services.
b. Increase access to residential alternatives with service supports for high risk mentally ill individuals.
c. Monitor and refine enhancements to the Assisted Outpatient Treatment program related to transition management, provision of medication and client tracking.
d. Expand access to a continuum of children's mental health and Wrap Around services designed to avoid or reduce out-of-home placement.
e. Increase the percentage of service provider contracts that are performance based.
